Stories from inside University Hospital Limerick, a hospital that had been called 'unsafe' and a death trap.
This Virgin Media News documentary examines the ongoing crisis at University Hospital Limerick through the stories of people who have been impacted by overcrowding and staff shortages at the hospital that, during an inquest, was referred to as a' death trap'.
Virgin Media News Correspondent Richard Chambers, speaks to three families who lost loved ones at UH, among them Karina Leahy (above).
Karina's father Pa, died at the hospital. She said: "'He never got the care he needed. In fact, he was just tortured from the minute he went in the door."
Speaking about the care her mother, Rachel received at the hospital, Ashlee Dickinson said: "You're trusting that your loved ones are going to be cared for as nest as possible . . . and we're not sure quite sure if that was the case."
Lucy Letby: Unanswered Questions – Panorama, 8.30pm, BBC One
After she was convicted of harming and murdering babies in her care, Lucy Letby (below) became one of the UK's most notorious child killers.
The nurse was found guilty by two juries after lengthy trials, but now a growing number of experts are questioning the prosecution evidence.
Judith Moritz, who has covered the case from the start, investigates the questions that have been raised about Letby's conviction.

He also reveals landscapes etched by giant dust devils, mountains coated with strange metallic frost and monsoon rains on a moon a billion kilometres from Earth.

Jamie Oliver prepares a simple sesame and miso salmon in minutes and fish cakes that can be made using store-cupboard staples.
He'll also be throwing together a gnarly lamb madras traybake that's prepped in minutes, leaving the oven to do the hard work.
And Jamie's youngest son, River, helps him to whip up a hot and crispy ice cream parcel
Love Cheats, 9.00pm, Channel 4
When firefighter James messaged Coleen and asked her out on a date, she was impressed by his charming, generous nature and fun-loving sense of humour.
She introduced him to her two daughters and her twin sister Karen and James soon becomes part of the family.
And when he reveals that he's expecting a £2million inheritance from his relatives in France, he goes into business with Karen, while planning an extravagant wedding for Coleen.
Despite James' claim that he's had a vasectomy, Coleen unexpectedly falls pregnant, and doubts begin to creep in about James' story.
As the deceptions start to unravel, the police investigate James' story, ultimately achieving some justice for the family and closure for Coleen.
